-Diagnosis and treatment of communicable diseases (this service is limited due to COVID-19 protocols at this time. Please call with questions)
-Dental screenings, education, and fluoride varnish application
-Vision, dental and blood pressure screenings
-Wart removal
-Wound evaluation and treatment, including some laceration repair
-Counseling services for a variety of needs to support students
-Classroom presentations
-Referrals for health care services which cannot be provided at the School Based Health Center 
-School Staff may be seen for immunizations

WHAT IS A SCHOOL BASED HEALTH CENTER?  A school based health center is a clinic located on a school campus operated by qualified mental and physical health professionals.
WHO WORKS THERE? The SBHC team includes a nurse practitioner, counselors. Medical staff provide comprehensive physical health services to help meet the holistic health care needs of students. Counselors support students as they return or improve to a higher level of functioning. Students can be affected by grief and loss, family issues, anxiety, depression, anger problems, problems with friends and school.
WHAT ABOUT APPOINTMENTS? Appointments are encouraged whenever possible. Please come by or call if you would like to make an appointment for yourself or a student.
WHAT ABOUT CONFIDENIALITY? Our health services and records are private, confidential and separate from school records.

WHAT ABOUT CONSENT? All students need to be registered with the SBHC each school year before services will be provided. To register forms can be picked up in person or mailed. If you would like to also receive dental services, please fill out the dental consent form. Paperwork can be returned to SBHC in person, by mail, or faxed. Once forms are received, services may begin at SBHC. 
OREGON STATE LAW requires a parent or guardian signature for mental health services for students under 14 years of age and medical treatment under 15 years of age, with exception of family planning information and sexually transmitted disease service. We support and encourage parental involvement in decisions about their student's health care.
SBHC SATISFACTION SURVEY  Students between the ages of 12 and 19 who visit the SBHC may be asked to complete an anonymous survey after their visit. The survey asks about the student’s satisfaction and experience at the SBHC, as well as some general questions about their physical and mental status. Students can refuse to take the survey and this will not affect their ability to get care at the SBHC. To see a copy of the survey, please go to the  SBHC Data Requirements link.
